Toe walking in children is a walking pattern where a child walks on the balls of the feet or toes without normally placing the heels on the ground. Occasional toe walking can be a normal part of early walking development, particularly in toddlers, but frequent or persistent toe walking after the early walking years may need professional assessment. Parents should pay attention if a child cannot comfortably walk with the heels down, has tight calf muscles, reduced ankle movement, poor balance, frequent falls, pain, stiffness, weakness, or walks differently on one side. Possible causes include idiopathic toe walking, tight calf or Achilles tendon, cerebral palsy, certain muscular disorders, and developmental or sensory factors. Toe walking can also occur in children with autism, but it does not mean that a child has autism on its own. Assessment usually includes reviewing the child’s walking history and examining ankle flexibility, calf tightness, muscle strength and tone, reflexes, balance, coordination, and walking or running patterns. Depending on the underlying cause, treatment may include pediatric physical therapy, stretching, strengthening, balance activities, gait training, ankle-foot orthoses, serial casting, and selected medical or surgical options when required. Home activities such as therapist-guided calf and ankle stretches, controlled heel-contact exercises, balance practice, strengthening, and slow heel-to-toe walking can support a professional therapy plan. PediGYM focuses on individualized pediatric therapy and ongoing progress monitoring to help children improve walking mechanics, ankle flexibility, balance, comfort, and functional mobility while addressing factors contributing to persistent toe walking. Early assessment can help identify whether the walking pattern is simply developmental or requires further evaluation and targeted treatment.
